Job Description

Forestdale currently seeks a Family Treatment Rehabilitation (FT/R) Case Planner who is responsible for working with families who are experiencing circumstances that threaten the safety and/or the removal of their children from the home. FT/R Case Planners works closely with families to promote healthy growth and development within the family. 

Major Responsibilities/Activities

  • Visiting families in their homes and at the agency as indicated by the Phase of Treatment — Initial, Baseline or Stabilization
  • Providing individual, family, and group counseling
  • Documenting all case activity with families and community providers using the N.Y.S. Connections system and N.Y.C. PROMIS system
  • Assisting clients with therapy, housing and daycare issues
  • Advocating for clients in their effort to secure services such as public assistance, health insurance, and help with immigration issues
  • Coordinating and maintaining contact with workers at the Administration for Children’s Services
  • Maintaining contact with schools, mental health facilities, and other service providers
  • Participating in staff meetings, Clinical Diagnostic Team meetings, in-service trainings, and outside trainings for professional development
  • Assisting in emergency situations as needed
  • Responsible for ensuring compliance with all agency policies, procedures and governmental regulations
  • Other duties as required by supervisor

Skill set

Minimum Requirements 
• Bachelor’s or Master’s degree in social work, psychology, criminal justice or counseling from an accredited college; related field
• At least two years of experience in case management, counseling and advocacy services, which may include field placement experience
• Strong organizational, written, and verbal communication skills
• Ability to accommodate a flexible work schedule
